Physical Waiver Form

WAIVER AND RELEASE FROM LIABILTIY FOR PARTICIPATION IN EXTRACURRICULAR ACTIVITIES.

This waiver and release is entered into by Student, Guardian of Student, and The Ranches Academy. Your son/daughter’s participation in The Ranches Academy Club is voluntary and not required as a student of the School.

1. Parent hereby gives consent for Student to participate and compete in The Ranches Academy Club.

2. Student and Parent acknowledge they are aware of the risks inherent in participation in The Ranches Academy Club.

3. Parent and Student hereby release and discharges The Ranches Academy, its officers, employees, agents and volunteers from all liability arising out of or in connection with the above described activity, that may be filed by, on behalf of, or for the above named student.

4. For purposes of this agreement, liability means all claims, demands, losses, causes of action, suits or judgments of any and every kind that arises from student’s participation in The Ranches Academy Club. The undersigned have read and voluntarily sign this release and waiver of liability and further agree that no oral representations, statements or inducements, apart from this agreement, have been made.
