Maturation Permission Form

Dear Parents,


On Friday, February 6th, the 5th and 6th grade students will be involved in this year’s maturation program.  Your permission, as stated in state law R277-474, is required.  We will not allow students without parental consent to participate in these sessions. If you consent,  please sign and have your child return this form to their teacher no later than Tuesday, February 3rd. 


We are pleased to have Carrie Ann Kemp presenting the maturation program to our girls and Brad Wilcox presenting to our boys this year.  Parents are strongly encouraged and welcome to attend. The presentation will begin at 10:15am for the girls and 10:30 for the boys. Both will end at 11:30.


The state mandates that this program be offered annually to 5th and 6th grade students.  Our feeling is that these issues are best discussed in the safe, nurturing environments of your own homes.  However, as a Utah public school, we are obligated to provide this program.  Students who do not have parental consent will have an alternate class made available for them or parents may check them out before the program begins.


We look forward to working with you to prepare your child for a healthy and successful future.  Please remember to return this permission form to your child’s teacher by February 3rd.
